Contrary to a widely published United Nations (UN) report that put the cholera cases and death tolls in Nigeria at 38 173 and above 1500 respectively, the Federal Ministry of Health (FMOH) has said that the country had 37 289 cholera cases and 1434 deaths between 1 Jan 2010 and 25 Oct 2010.

The statement added that laboratory results of stool and water samples collected from suspected cases indicated that Vibrio cholerae [コレラ菌] was isolated from Gombe, Adamawa [アダマワ州], Bauchi, Borno, Jigawa, and Taraba States, while coliform was isolated from water samples in Rivers State.

国内のほとんどの地域を豪雨が襲い、多くのコミュニティーで洪水が発生したことが、大部分の州、特に北東部でコレラが発生した理由である。

It was also said that the reason for the outbreaks of cholera in most states particularly in the north eastern part of the country this year [2010] was as a result of heavy rains in most parts of the country leading to flooding in many communities.

The Vibrio cholerae strain responsible for the expanding cholera epidemic in Haiti is nearly identical to so-called variant seventh-pandemic El Tor O1 strains that are predominant in South Asia, including Bangladesh.

Earlier in Nov 2010, a Collier County woman [フロリダ州コリアー郡の女性。下記 ProMED-mail Archive Number 20101117.4171 参照] who had returned from Haiti was found to be suffering from the disease. She has recovered. A 2nd case is suspected but has not been officially confirmed.

入院患者の致死率 2.6%, 全体の致死率 2.1%.

Mortality varies between departments, the highest being in Grande Anse [南西部 グランダンス県 人口10万人あたり 130.5人], notably higher than the rest (130.5/100.000) and the lowest being in the Ouest [首都圏 ポルトープランスを含む西県 人口10万人あたり 14.4人], (including Port-au-Prince ), of 14.4 deaths per 100.000 inhabitants.

On November 17, the MSPP (Ministère de la Santé Publique et de la Population [保健・人口省]) reported that between October 19 and November 15 the cumulative number of hospital admissions and deaths due to cholera was 18,382 and 1,110, respectively. Seven of ten departments [全10県中 7県] are now affected (Artibonite [アルティボニット県], Central [中央県], Ouest [西県], Nord Ouest[北西県], Sud [南県], Nord [北県], and Nord Est [北東県]).
In Port-au-Prince [西県/首都 ポルトープランス], the numbers of cumulative hospital admissions and deaths were 953 and 46, respectively, for the same period. In the metropolitan area [ポルトープランス 首都圏], cases of cholera have been confirmed in six of seven communes [最小行政単位 コミューン]

The strains of Vibrio cholerae 01 Ogawa isolated in Haiti and tested by the National Public Health Laboratory (LNSP) and the US CDC showed resistance to the following antibiotics:
trimethoprim-sulfamethoxazole, furazolidone, nalidixic acid, and streptomycin. The strains are sensitive to tetracycline, doxycycline, and ciprofloxacin. Full genetic sequencing of the strains is ongoing.

Cholera was confirmed on Fri 22 Oct 2010 during the morning by the Haitian health minister, Alex Larsen, who said that this is cholera type O1 [コレラ毒素産生 O1型], which is the most dangerous and the source of most outbreaks of cholera in the world, according to the World Health Organization (WHO).

[Mod. LL] The organism is reported to be O1. O1 and O139 are the only strains able to cause epidemic spread of cholera. The other markers of identification, the strains’ biotype and phenotype, are pending.

Infectious Disease Impact Scale (IDIS)
We have indications of an infectious disease event in Haiti (Artibonite Valley) rated now at a possible IDIS Category 4 infectious disease event transitioning to a Category 5, defined as:

IDIS Category 4. Infectious disease event associated with social disruption. Category 4 events highlight when organized response has occurred, yet significant social disruption has been documented.

IDIS Category 5. Infectious disease event associated with disaster indicators.

Key observations as of the date/time of this message:

-Non-routine occurrence of diarrheal disease, described by Dr. Claude Surena, President of the Haitian Medical Association, as “according to the results of the analysis carried out in the laboratory it is cholera” to AFP
