Subject PRO/EDR> Malaria – Haiti: post earthquake
Haiti [公用語フランス語では発音 アイティ] 政府によると、地震による死者は約 200 000人で、500 000人が家を失った。
マラリア原虫 Plasmodium falciparum による熱帯熱マラリアはハイチでは風土病であり、主な媒介蚊 Anopheles albimanus [ハマダラカ]は屋外で刺すことが多い。
2010年1月12日から同年2月25日までの期間に、同国で熱帯熱マラリアに感染した確定患者は 11人(緊急要員の在住アメリカ人 7人、ハイチ人 3人、米国人旅行者 1人)。
在住アメリカ人のうち 6人は米国の軍人で、このうち 4人は合併症がなかった[ので]、ハイチで治療を行った。他の2人は、中等度から重度で集中治療のため米国に移送となった(1人は急性呼吸促迫症候群 [ARDS] のため、気管内挿管と人工呼吸器が必要であった)。全患者が完治すると予想されている。
軍人 6人は米国出発前に経口予防抗菌薬 ドキシサイクリン doxycyclineを与えられ、個人用保護具 [PPE]として、防虫剤、殺虫剤処理した網や服はハイチ到着後にもらった。
全11症例中、マラリア予防の化学療法は 緊急要員 7人および旅行者 1人[計 8人]に指示されたが、このうち 入院した軍人2人を含む 6人は推奨されたマラリア治療薬[の服薬]を遵守していなかった。他2人の[服薬]遵守の状態は不明。
ハイチ人の住民 3人(一人は養子)は米国に旅行した者で、ハイチから米国に輸入されたマラリア患者の人数は、全例がCDCに報告されていないので、過少評価されている。

毎年、ハイチからのマラリア確定患者の報告数は約 30 000人であるが、年間 200 000人もの患者が発生している可能性がある。2006年のArtibonite Valley(ハイチ首都 ポルトープランスの北 75マイル)での地域住民調査では、熱帯熱マラリアの有病率は、全体で 3.1% (熱のある住民の 14.2%, 平熱の住民の 2.1%)であった。
Although each year Haiti reports approximately 30 000 confirmed cases of malaria to the Pan American Health Organization, as many as 200 000 cases might occur annually. One population-based survey in 2006 in the Artibonite Valley, located 75 miles north of Port-au-Prince, found an overall prevalence of _P. falciparum_ infection of 3.1 percent (14.2 percent in febrile and 2.1 percent in nonfebrile persons) (4).
